HSE INSPECTION & HSE AUDIT

HSE inspections & Audits

Different kinds of HSE inspections and audits shall be conducted for this project, the inspections and audits are summarized as follows:

·         HSE Walkthroughs 

·         Client or its delegate's HSE inspection

·         Company corporate HSE inspection

·         Project management site tour.

·         Project-specific HSE inspection (equipment safety inspection, scaffold inspection, electricity inspection, adverse weather impact inspection, etc.)

Planned Inspections

The Company Project management department shall conduct planned HSE inspections, the inspections may include but are not limited to the following.

·         HSE personnel daily regular site HSE inspection

·         Weekly walkthroughs with project Management

·         Equipment prequalification inspection and quarterly safety inspection

·         Specific HSE inspection, for example, Electrical inspection, scaffold inspection, lifting gear inspection, etc.

HSE observation reports

All HSE inspection observations shall be corrected properly timely, and the observations shall be logged on a register. A safety analysis shall be conducted for the registered HSE observations to identify the current main HSE concerns, based on the observation analysis result, the specific safety control measures shall be taken accordingly. All HSE observations shall be documented and maintained properly by the HSE department.

HSE meetings between the company and its Subcontractors shall be organized as necessary while implementing the project monitoring program, for instance: the HSE audit and inspection kick-off meeting and close-out meeting, etc.

HSE Inspections

The project HSE team, including subcontractors, will carry out daily site inspections covering project site active and support areas. Inspections must cover all site aspects including but not limited to access and egress, emergency arrangements, ongoing activities, working platforms, environmental aspects, interface with public and/or other contractors, welfare facilities, stores and laydown areas, tools and equipment, electrical installations, permits to work, operatives qualification and competencies, displayed notices, use of PPE, etc.

HSE inspection findings shall be communicated on the spot with the concerned parties and corrective actions shall be taken immediately whenever possible. When immediate corrective action is not possible, the inspector shall evaluate the risk and act upon it. For low-risk findings, the inspector shall agree with the concerned party on a reasonable timeframe to take the required actions and shall monitor the progress to ensure compliance with the agreement and follow up when and where required. Repetitive low-risk findings shall be escalated and treated as medium risks. When a medium risk is observed and immediate corrective action is not possible, measures shall be taken to reduce the risk or the consequences as low as reasonably practical until corrective actions are taken. If the corrective actions can’t be taken within the same working shift, the inspector has to report the situation to the project HSE manager who shall evaluate the risk and decide whether the work may proceed or not. When a major risk that presents an imminent danger is observed, work shall stop immediately, people shall be removed from the danger zone, and appropriate measures shall be taken on the spot to avoid other people from entering the danger zone until the required measures are taken, the area is safe and the work process doesn’t present a risk to people, properties and the environment.

Medium and high-risk Inspections’ findings shall be discussed during the HSE meetings and toolbox talks to reduce the chance of its reoccurrence.

Inspections’ findings shall be recorded using the “HSE Inspection Report” and records shall be maintained by the HSE department.

In addition to the general HSE inspections, specific inspections shall be conducted regularly with items or discipline targets including, but not limited to, equipment, tools, lifting gears, personal fall arrest systems, electrical connections and installations, ladders, scaffolds, excavation trenches, etc.

HSE Walkthrough

The project Management team shall, regularly, conduct site HSE walkthroughs where they target one focus area /zone and cover all HSE aspects in it. The walkthrough shall include communication with the teams working in the area, advising them on safe working practices, and getting their feedback.

This walkthrough must be recorded including the date and time, participants, areas/zones covered, findings, feedback, corrective actions, preventive actions, responsible persons, and close-out timeframes. The project HSE Manager has the responsibility of distribution and maintenance of the records as well as following up with all parties who are required to take corrective and/or preventive actions.

Audits

The project will be subject to internal and external audits. Internal audits will be conducted by the HSE department where the project documentation as well as the project sites will be thoroughly audited to ensure the project compliance with the corporate HSE management system.

External audits will be conducted by a third party as part of the OHSAS 18001:2007 certification process.

Audits reported will be sent to the project management team who shall take the necessary actions to close out all non-conformances within the given timelines.
